Reporting a Medical Facility or Hospital
The Illinois Department of Public Health (IDPH) investigates reports of harm to patients. It also investigates harm due to unsafe building environments. If you or a loved one has been harmed or could have been harmed at a hospital or other medical facility you should file a complaint with IDPH. You can do so by any of the following:

Reporting a Doctor
If you have been harmed by a doctor, in addition to filing an IDPH complaint, you should also report the doctor to the State Medical Disciplinary Board. Often times, a victim of a doctor’s negligence leads to a medical malpractice case. If this applies to you, we can help.
To file a complaint against a physician, you need to file a complaint with the Illinois Division of Financial and Professional Regulation (IDFPR), and more specifically, the Division of Professional Regulation. You can file a complaint by contacting the Complaint Intake Unit. There are two ways to file a complaint, online or by printing out and filling in the complaint form.
